How do I upload my student's Physical and/or Medical Documents?
If there isn't an option to upload your forms, your district may require in person delivery. Only schools that enable a “Physical Upload Form” on their online portal allow uploads. For other issues, please verify all fields are completed, or check with the athletic office for approval details.
1. Check for a “Physical Upload Form”

Some schools require physical forms to be submitted directly to the athletic department rather than uploaded online.

Visit your school’s online forms site and look specifically for a “Physical Upload Form.”
2. If no upload option is available

You may be required to deliver the physical forms directly to the athletic department.

3. If your form is rejected or you continue to see errors

Verify that all required fields and signatures have been completed.
