I'm having trouble submitting forms online and want to print them instead. How can I access blank copies?

We do not offer blank copies of forms for users who prefer to not submit electronically or have trouble doing so, unfortunately. If you wish to print blank copies of any electronic forms, you will need to print your entire screen. 

1. Print from the Parent Portal

  • Open the electronic form in the Parent Portal, then right-click and select “Print,” or choose the option to save the file as a PDF.

2. Submit to the School

  • After completing any forms by hand, submit them to the athletic office or fine arts office at your student’s school.

  • School staff will document the forms as received in Rank One.

3. Download & Print Section

  • The documents available in this section are the forms your school requires you to print, complete, and submit manually. Common examples include Physical Examination and Medical History forms.
